STUDENT TRAINING AGREEMENT

1. Program Overview

BenchTestPrep, led by Dr. Shalini Thasma, BDS, DDS, offers hands-on dental training through

two main programs: the 4-day and 6-day Bench Test Training for Advanced Standing Program

aspirants, and the 4-day and 6-day ADEX/CDCA-WREB-CITA Licensing Exam Training.

Training is conducted in person at a dental facility in Frisco, Texas. This program is not affiliated

with any specific university, but is designed based on the commonly observed requirements of

university bench exams and follows the ADEX requirements.

2. Student Responsibilities

Students are required to review the curriculum and course structure on the BenchTestPrep

website before attending the training. It is the student’s responsibility to ask questions and seek

clarification on any course content or procedures. Students are expected to practice their

assignments independently and should not rely on constant supervision throughout the training.

3. Attendance and Registration Policy

Enrollment into the program is accepted on a first-come, first-served basis. There are no refunds

for missed sessions, regardless of the reason. Each training day begins with a demonstration

that lasts approximately two to three hours, followed by a practice session that may last five to

six hours. Students must attend all scheduled program days for which they are registered to be

eligible to receive a certificate of completion.

4. Participation Guidelines

All materials required for the training, including the Kilgore typodont, additional Kilgore teeth,

handpieces, hand instruments, burs, and filling materials, are provided to students on a rental

basis. It is the student’s responsibility to return these materials in proper working condition at the

end of the course. If any instrument is damaged, such as a hatchet, the student will be charged

for its replacement. Students are welcome to bring their loupes and typodonts. Burs and

instruments may also be purchased on-site or through the website.

The ADEX/CDCA-WREB-CITA training encompasses the Restorative, Prosthodontic, and

Endodontic components, but excludes the Periodontal section. Practice is conducted using

Kilgore typodonts, Kilgore restorative teeth, and Kilgore endodontic teeth.

Demonstrations will be performed by the instructor, Dr.Shalini Thasma, on one tooth per

procedure, and students will be assigned additional teeth to practice the same preparation.

For example, if a Full gold crown preparation is demonstrated on tooth number 19, students may be assigned to perform the same preparation on teeth number 30, 18, 3, and 15 to reinforce their

learning through varied repetition.

Students are expected to complete daily preparation assignments independently and present

each completed task to the instructor for real-time feedback. This process allows mistakes to be

identified early and corrected quickly, which helps build confidence and independence. Students

are encouraged to take initiative in their learning and are expected to actively engage in practice

rather than rely on continuous guidance throughout the day.

5. Code of Conduct

All students are considered beginners regardless of prior clinical experience, and the training is

designed to teach ideal preparation techniques from the ground up. Students are expected to

respect their peers, the instructor, the staff, and the training facility. Instruments and materials

should be handled with care, and students should avoid wasting these resources. Students

must follow all instructions provided by the instructor and staff. Yelling, arguing, or behaving

inappropriately toward instructors, staff, or fellow students will result in immediate dismissal from

the program, without a refund. It will also deny access to videos and continued support.

Students should wear clinical attire, including scrubs or comfortable clothing with closed-toe

shoes. Shorts and sandals are not permitted. Photography, audio recording, and video recording

are strictly prohibited during class and live demonstrations.

6. Violations and Disciplinary Actions

Failure to comply with the rules outlined in this agreement may result in the student being

removed from the course without a refund of tuition. In such cases, students will also lose

eligibility to receive a certificate of completion.

7. Confidentiality and Intellectual Property

All training materials, instructional content, and demonstration formats are the intellectual

property of Dr. Shalini Thasma, Benchtestprep, and Coit Dental Academy LLC. Recording,

duplicating, or distributing any part of the course content without written permission is not

permitted. The course format, content, and modules can be changed at any time and are at the

sole discretion of the course director, Dr. Shalini Thasma BDS, DDS.

8. Clinical Practice Disclosure

BenchTestPrep training is conducted in a private dental office setting. While the primary focus

during training days is instruction and hands-on practice, the instructor may also perform patient

care at a limited capacity during the course. This clinical work will not interfere with the training

schedule. The instructor remains available throughout all training days to provide

demonstrations, feedback, and individualized guidance.

9. Payment and Cancellation Policy

All registration fees are non-refundable and non-transferable. If a student submits a cancellation

request more than seven days before the course start date, a refund will be issued minus a

$500 cancellation fee and any PayPal processing fees. If the cancellation request is made

within seven days of the course start date, no refund will be issued. Students who withdraw from

the course after it has started or who fail to attend any portion of the course will not receive a

refund. A student’s request for a date change may be considered at the sole discretion of the

course director. Because this course is conducted within a working dental office, cancellations

with short notice negatively impact the ability of other students to participate and also disrupt

clinical workflow.

10. Certification and Results Disclaimer

A Certificate of Training will be issued only to students who complete the 4-day or 6-day Bench

Test Training programs. No certificates will be issued for the ADEX/CDCA-WREB-CITA licensing

exam training courses. Continuing education credits are not provided for any program.

Participation in this course does not guarantee admission into any dental program. Success in

bench tests depend on each student’s individual preparation, practice, knowledge, and skill

level.

11. Health and Safety

Students should be aware that artificial teeth can generate fine dust during preparation. For their

safety, students are strongly advised to wear personal protective equipment such as gloves,

masks, and safety glasses. BenchTestPrep, Dr. Shalini Thasma, and Coit Dental Academy LLC

assume no liability for health issues that may arise during or after participation in this training.

12. Consent and Emergency Contact

By signing this agreement, the student grants permission for photographs and videos taken

during the training to be used for promotional and educational purposes on platforms such as

Facebook, Instagram, and YouTube, unless the student submits a written objection in advance.

The student also agrees to provide emergency contact information before the first day of

training.

13. Acknowledgment 

I have read, understood, and agree to abide by all the policies and responsibilities outlined in

this agreement.
